In-home Health Assessments

KWMC offers Health Assessments in the home for patients over 75 years of age. These assessments are very much encouraged, as they provide a comprehensive review of health. Our registered nurse visits the patient in their home to assess their nutrition and vision, their safety in their home environment, and their current health needs. KWMC is committed to the continuity of health needs.

GP Management Plans & Team Care Arrangements

We provide GP Management Plans for patients with complex needs, and these plans incorporate the various allied health services in the community. Team Care arrangements are available for select patients.

Medicare offers five services per year for those patients who require allied health services, to assist in the management of their chronic disease conditions.

The Health Record Exchange programme, which is administered via the local division of General Practice, can provide additional services for these patients.

Diabetes Management Reviews

Diabetes Management Reviews provide a comprehensive annual check on patients with this condition.  Our registered nurse, Cathie, conducts diabetic clinic every Monday afternoon.  Those Patients who are confirmed Diabetics can make an appointment to attend Cathie's clinic.

Asthma Management Plans

Asthma Management Plans prepare patients for the overall management of this condition.
